RETURNING GRANT APPLICANT
- Show Hope Medical Care grants are to be used for medical care or medical equipment (see "Eligible Expenses" below) for children who have joined their families through adoption.
- Show Hope Medical Care grants are non-transferrable and for the sole purpose of covering medical care or medical equipment expenses for the child named on the application. Medical services covered by grant funds must be completed before the child turns 18 years old.
- An additional application should be submitted if another child in your family has medical needs that are eligible for funding.
- We are not able to award grants to families unless, at least, one parent is a U.S. citizen.
- The child must be receiving medical care in the U.S. from a licensed medical provider.
- The family must have incurred medical expenses for their child’s treatment within the prior 12 months of the application's acceptance date or have anticipated out-of-pocket expenses for treatment within the 24 months following the grant award date. The incurred date is the date of service the medical procedure occurred or date the medical equipment was purchased, not the date the payment was made for the service.
- Funding is not available for clinical trials, medications not approved by the FDA, or for medical treatment that does not align with Show Hope’s Statement of Faith.
- Funding may be utilized for medical care, including procedures; surgeries; hospital stays; treatments administered by a licensed medical provider; medical equipment; and rehabilitative speech, and physical therapy.
- Medical equipment needs that are eligible for possible funding include wheelchairs, stand-alone wheelchair and patient lifts, and/or medical equipment or devices associated with a specific diagnosis as recommended by a licensed medical provider.
- Certain expenses may be eligible but will be reviewed on a case by case basis:
- If occupational therapy is being pursued to help address a known medical diagnosis as confirmed in writing by a licensed medical provider, applications will be considered on a case-by-case basis. Examples include bone fractures, burns, cerebral palsy, spina bifida, traumatic brain injuries, etc.
- If cognitive rehabilitative therapy is being pursued to help address a traumatic brain injury or illness that impacts brain function, as confirmed in writing by a licensed medical provider, applications will be considered on a case-by-case basis.
- If dental procedures are necessary to treat a medical diagnosis or additional services are required to accommodate underlying health conditions, applications will be considered on a case-by-case basis with written confirmation from a licensed medical provider. In general, preventative dental care, orthodontics, and cosmetic dentistry are not eligible expenses.
- If travel outside of one’s home city or state is necessary for a child’s medical appointments, then travel and lodging expenses will be considered on a case-by-case basis.
- Home and vehicle modifications
- Behavioral health expenses. Examples include ADHD, autism, brain integration therapy, counseling, neurofeedback therapy, etc.
- Service dogs or service dog training
- Before beginning the application process, please review Show Hope’s Statement of Faith.
- Collect the following pieces of information:
- Church address, phone number, website, and pastor’s email address
- IRS tax information (1040 or 1099 and W-2 forms) for the past two years (please black out social security numbers)
- Current Year Pay Stub
- Net worth of property, investments, goods, and liquid assets
- Insurance summary of benefits and coverage document
- Monthly claims statement or EOB showing deductible/OOP max and amount met
- Final Decree of Adoption
- Family photo (optional)
- Identify both your medical provider and pastor references. It's helpful to let them know beforehand they will receive a reference request from Show Hope.
- Applications for each child cannot be submitted:
- For medical expenses for which an application has already been submitted
- Within two years of previously submitting a Show Hope Medical Care grant application regardless of award status

Behavioral health, service dog, home and vehicle modifications, preventative dental care, orthodontics, and cosmetic dentistry are not eligible expenses at this time. Families can apply for a Medical Care grant multiple times for one child up until that child is 18 years old. There is a two-year waiting period between grant awards per child (once previous grant funds have been completely exhausted). Families can apply for multiple children at the same time; however, a separate application for each child will need to be filled out.
If dental procedures are necessary to treat a medical diagnosis or additional services are required to accommodate underlying health conditions, applications will be considered on a case-by-case basis with written confirmation from a licensed medical provider. In general, preventative dental care, orthodontics, and cosmetic dentistry are not eligible expenses.
Unfortunately, behavioral health services including counseling services are not eligible for Show Hope Medical Care grants at this time. If cognitive rehabilitative therapy is being pursued to help address a known medical diagnosis, as confirmed by a medical provider, applications will be considered on a case-by-case basis.
No. Your child’s adoption must be finalized state-side prior to applying for a Show Hope Medical Care grant.
All grant funds expire 24 months after the award date unless an extension is requested in writing. Once a Medical Care grant is awarded, Show Hope reserves the right to determine if any major amendments to the medical care plan occurred or if any issues were raised that would disqualify the applicant from receiving a grant.
Due to Show Hope’s 501(c)(3) status and IRS tax regulations, monetary gift donations cannot be designated to a specific child or family who has been awarded a Medical Care grant.
Yes! If you are a recipient of a Show Hope Adoption Aid grant, we encourage you to apply for a Medical Care grant once your child’s adoption has been finalized and if your child’s medical needs meet our qualification and eligibility requirements.
Yes! Medical Care grants can cover a vast array of medical needs, even needs that have arisen long after a child has come home through adoption. Show Hope’s preference is to pay a medical care provider directly for services received by a family. If payment has already been made by the family to the medical care provider, Show Hope can reimburse the family’s expenses with proof of payment and an invoice/receipt from the provider.
